Hyderabad flyover collapse: engineering gone bad?

The recent collapse of a Flyover under construction at Hyderabad is indeed a wake-up call for all those who are involved in building and maintaining flyovers. It also underscores the need for active watchdog efforts from citizens that could have prevented this and other similar disasters. I received a PPT file with a few photos of the section that collapsed. (See attached file).
The source is unknown (please do let us know if we can credit someone for this). The annotations are all in the original file.
The bottom line is that the design and/or construction of the flyover was not robust. Very basic laws of engineering design were violated.
I am no expert in structures and I cant say for sure if this is a conspiracy theory or has some truth to it. May be the readers can throw some light on this.
