Doing a survey for bus day, help please

Task to create a not too geeky, not too simple survey that we will run online (and if possible, offline as well) till the bus day.
I am thinking he survey would consist of 2 or max 3 simple questions. Ignore the verbiage and text here, will make this less geeky.
1) Mention your top 2 reasons for not taking the bus
- Bus slower than my car/bike (speed)
- Bus stand too far from office/home (last mile)
- Don't have a direct route to office, and very hard to change the bus (interchanges, routing)
- Bad perception of BMTC drivers (Killer BMTC perception)
- Buses are crowded and dirty (comfort)
- Tickets are expensive (cost)
- Too few buses on my route (frequency, routing)
2) What type of service do you use, or prefer to use (list types of services)
- Regular/Atal Sarige
- Pushpaks/Suvarnas
- Big10
- Vajras
What else. Is this simple enough?

SB,
I tend to agree that a 2-3 question poll-like survey can attract lots of participants. lets have it.
But, lets also have a detailed survey. We can work out specifics/details later. Lets see if the poll results are a good summary of the summary results.
Anyway, a survey will have many more parameters and also many dimensions of view. We can think of a few contests and get people to participate. The contests can be like
- if you answer the survey closet to the overall public, you are in for a prize
- Invite most number of people for the survey and get a prize ( Invite people by marking email to a contest id / build a feature within the site to invite people for survey)
I will take a second/third look at the survey link posted by Bharat ( was it posted earlier on Praja?) and provide with a set of questions.
Unreliable Bus Service!

Basically, can adda category around predictable timings information on schedule etc.
Lets limit this to max 10 items, from which we will ask respondents to pick top 2 or 3.
Take the Bus Campaign - Poll Questions!

In today's skype call, Sri had very good suggestion on the question of approach to doing online survey.
The suggestion was to have the Poll survey before the event and post event comprehensive survey.
- Pre-Event Poll - Idea here is every week we run a poll asking questions centered around 'Take the Bus' event. As weeks progresses, the poll questions will get added to the list. The polls will appear on campaign page home page and also on Praja home page. The older questions would still continue to be open and running.
- Poll questions -
Week # 1 Question - Would you be taking the BUS to work on Feb 4th?
- Yes - I take the bus daily to the work
- Yes - I will take the bus on Feb 4th
- No.
Week # 2 Question - What is the one reason for you to say 'NO' to BMTC Bus for daily commute?
- Expensive Ticket Price
- No direct route
- Too few buses on my route (frequency, routing)
- Very hard to change the bus (interchanges, routing)
- Unreliable Service and schedule
